So the modem is in 'sync' with the line card at the exchange - this

OAM is an Operation, Administration and Maintenance packet to test the 
ATM (Asynchronous Transfer Mode - the transmission system used over the 
ADSL link).  The fact it fails isn't necessarily a problem; it could 
just be that the ISP's kit doesn't support / blocks ATM OAM requests.

Since this failed, the rest has to fail.

What's your ISP?  Do they definately use PPoE, or could it be PPPoA?

Do you have the correct VPI + VCI values?  (These are the path + circuit 
  identifiers; here in the UK they're 0, 38 - but they may vary where 
you are.
